Understanding Therapy Rates and Insurance
We want you to feel informed and supported when starting therapy. Here is a clear overview of our therapy rates and insurance coverage, self-pay options, and what to expect before your first session at Dr. Brecke Counseling Group in Eagan MN.
Quick Facts
- Insurance: We accept most major insurance plans, including Optum (Medica, UMR, UBH, UHG, UHC, Surest), HealthPartners, Cigna, BCBS, Aetna, and First Health.
- Self-pay: $175–$210 for a 53–55 minute session depending on provider and level of education
- Payment: Debit card or major credit cards, HSA/FSA cards
- Cancellation: 48 hours or 2 full business days are required to avoid a $100 late cancel or no-show fee
Self-Pay Rates
Each clinician sets their own self-pay rate based on experience and session length:
- 35–45 minute sessions: $140–$165
- 53–55 minute sessions: $175–$210
You can find each clinician’s exact rate on their bio page. Meet our providers.
How Insurance Coverage Works
Every insurance company has a contracted rate agreed upon with our clinic. These rates are usually similar across mental health clinics in the area because of standardized fee schedules.
Common Billing Codes
- 90791: Diagnostic assessment, usually 1–2 sessions at the start of treatment
- 90837: Ongoing individual therapy, 53–55 minutes. The number of sessions varies widely based on diagnosis, history, treatment goals, and participation.
Because codes depend on the type and length of service, the exact amount billed isn’t known until after the session.
Typical Contract Rate Range
Contracted rates for a 90837 session generally fall between $165 and $225, depending on your insurance plan.
Your Out-of-Pocket Costs
Co-Pay
If your insurance card lists a mental health or primary care co-pay, that is the amount you will owe at each session.
Deductible
If no co-pay is listed, you likely have a deductible to meet before your plan pays for services.
Plans With No Client Cost
Some plans cover mental health services at no cost to the member, including:
- Hennepin County employee plans
- Minnesota State employee plans
- MNCare / PMAP Medicaid plans administered by the state
Accepted Insurance Plans

We accept most major insurance plans, including Optum (Medica, UMR, UBH, UHG, UHC, Surest), HealthPartners, Cigna, BCBS, Aetna, and First Health. Here are the details of our in-network plans:
- Optum plans: Medica, UMR, UBH, UHG, UHC, Surest (no state Medicaid, Medicare, or MN Fairview plans)
- HealthPartners
- Cigna
- Blue Cross Blue Shield (BCBS): all plans; out-of-state plans are processed through the local plan
- Aetna: no Medicare or Allina plans
- First Health
- Sanford Health
- EAP: we accept EAP plans from Optum and its subsidiaries only
Plans We Cannot Accept
We are out-of-network with any plan not listed above, as well as:
- Medica State (Medicaid)
- Medica Medicare
- Medica Fairview
- Allina plans through Aetna
- Aetna Medicare
- Aetna Mayo
- All Employee Assistance Programs except Optum
If you’re unsure whether your plan is in-network, please call your insurance company to confirm coverage.
We don’t contract with some plans because they don’t offer a fair rate for the experience, education, and specialty training our clinicians bring. To maintain the integrity of our work, we only contract with plans whose rates fairly cover the service.
How to Verify Your Insurance Benefits
We strongly encourage you to call the number on the back of your insurance card before submitting an appointment request. Knowing your benefits ahead of time can reduce financial stress and help you plan your care.
Questions to Ask Your Plan
- What is my deductible, and how much has been met?
- Do I have a co-pay, and what is the amount?
- What is my coverage per therapy session (codes 90791 and 90837)?
Tip: Write down the representative’s name, the date and time, and the details of the call in case claims are denied later.
Good Faith Estimates
If you’re concerned about cost, your clinician can create a Good Faith Estimate based on your insurance contract rates. This is an estimate only. Your insurance company is the only source that can provide a binding cost breakdown.
Billing and Claims
We submit claims to your insurance as a courtesy. Please be aware that:
- Your insurance company may deny claims or not process them correctly according to our contract.
- You are responsible for any balance not covered by insurance.
- We will notify you if a claim is denied, and you may appeal denied claims directly with your plan.
- We guarantee we will reprocess any claim rejected because of a clerical error on our part (for example, an incorrect date of birth or address).
- We guarantee to submit a clean claim, meaning all the information you gave us, including your diagnosis, billing code, and date of service. If the information you provide is incorrect, the claim will not clear for payment.
- If a claim is rejected for a reason we can’t fix, such as “we cannot identify the member,” you will need to call your plan. You can always switch the claim to self-pay instead.
- Sessions may be paused at the discretion of the therapist, billing department, or owner if insurance is not paying and the client is not willing to switch to self-pay. Accounts must stay under $200 to continue scheduling.
If insurance pays after an appeal, we will refund any resulting credit.
Payment Methods and HSA/FSA
We accept debit cards and all major credit cards, HSA/FSA cards. You can use HSA or FSA funds for co-pays, deductibles, or self-pay sessions, which many clients find helpful when their plan is out-of-network or coverage is limited.
Cancellation Policy
If you miss a scheduled appointment or cancel with less than 48 hours’ (two full business days’) notice, a $100 late cancellation or no-show fee will be charged. This fee cannot be billed to insurance.
